Why does some cover art keep disappearing?

I've noticed this for quite a few months now and I have no idea why it's happening.
I have a library that contains a load of BDs (ripped to the folder structure) and DVDs (ripped as mkv). BDs are ripped by mediacenter and the "get tv/movie info" feature is used to provide metadata. The data is ripped to a mapped network drive and everything seems to work just fine except I've noticed that a small number (2-3 per month?) BDs lose the cover art. No other metadata disappears, just the art. The behaviour is not BD specific, it is seemingly at random as to which film loses the image.
Does this sound like a familiar problem to anyone? Any pointers as to options to check? I'm not sure what other info to provide here.

Do the cover art JPG files disappear too? Or just MC's link to the cover art? I.E., can you resolve these by doing Cover Art > Quick Find in file/cover art directory ? Or, can you see the JPG file in the same directory with the movie file?
I'm not sure what would cause either behavior, but it seems important to know if the actual cover art file itself is being removed somehow.

As far as I have noticed, when using "get tv/movie info" on a BD ripped to to disk, jriver puts a file inside the ripped directory structure that contains the image. This file seems to go missing. NB: I was expecting those files to be in the Cover Art directory but they don't seem to be.
Nothing else, apart from the library server, touches that disk with write permissions. There are no users floating around doing random stuff on the OS either.

ha someone else we know just posted a problem with his thumbnails ;)... just curious to see if this happened to you and Matt ... check Options > Tree & View > Thumbnails > Create thumbnails for videos .. did this gets switched off for some reason too??
Your BD cover art should be in the folder with the film .. regardless of the options .. this was also brought up by someone else so that shouldn't be the problem

Cover Art > Quick Find doesn't do anything
I took a directory listing of a BD that has this problem, then ran "Get Movie & TV Info" and then repeated the listing. Here's the diff

to be clear, these are old files that haven't been touched by anything else in ages.
