I have an album that has been recorded at an unacceptably low level.  How can I modify the file and save it with an increase in volume?

You could just use the converter and specify a volume increase.

Or you could use Audio Analysis and Volume Leveling.  This is probably even better.

An external tool like Audacity (free) would do the job. 

Like Matt, I like the idea of just doing it with Volume Leveling, which runs real time to alter the playback volume of all tracks to make them similar in overall perceived volume.

Both ways have merit.

Scan your library with the audio analyzer. Then enable volume leveling.

Do any of these actually modify the file so if I export to an iPhone I get the new settings?

if you use the handheld sync tools, you can force volume levelling and DSP data into the tracks on the fly so that they aren't "permanently modified" on the computer but are on the playback for your phone (i use this a lot)

you can also convert (lossless, like flac or alac) right back into the flac or alac and force in the DSP data if you like

r128 (volume) data sits inside tags, so many devices and other programs can read those as well.
