wait ... before you try what I suggested before ... try this first (one guy reported it was working for him)

set your destop to 4k@60 and enable 3D in both windows and nvidia cpl

leave madVR setting at default ... then set follow

[rendering -> stereo 3d]
enable stereo 3d playback: check

[rendering -> general setting]

check use Direct3D 11/present frame for every VSync and delay playback start until rendere queue is full (this crashes my MC when playing 2D stuff ... but it worked for other guy)

How did you rip a UHD movie have I missed some development?

I'm using makeMKV to rip and it has now been able to rip all of my UHD's

I only tried playing 2D with this configuration and my system went into a tailspin. Multiple madVR resets, Windows restarts, and an MC re-install later I have 2D working again.

I'll try your other option when I get the chance.
