Expression used to work and now it doesn't?

I got GREAT help a few years back on this expression to rename my pics by date and camera model. It has worked flawlessly for years, but now has weird results, like usually a generic date of 01-01 with no year, and the time is 00-00-00. I don't know what happened because I copy and paste the expression every time I have to reinstall MC so it should be identical.

I included a screen shot. The old filename and the new filename *supposedly* used the same expression, but obviously something has changed.

I would love to figure out what happened...

I just tested this on some random image files, and all returned expected results. I am running MC28.

Please confirm which version of MC you are currently using.
Double check that the date field is correctly filled. Appears to be a dumb suggestion, I know, but often it's the most obvious things that we miss :)
