How does one completely purge/reload a (video) library?

I've read the wiki articles, but am still not clear on the process.
Hoping this will fix/cleanup multiple missing and incorrect listings.

Running MC on Windows 10 and networked (wired) to a NAS as the only source for video.

Does File > Library > Clear Library not work? Probably a good idea to do a library backup first before trying that.

Sort of works.  The old Library was, indeed, cleared out.
The Reload improved a few mis-named titles, but left many incorrect and/or incomplete.

My frustration stems from the fact I do not understand how it can correctly read some folders of DVDs ripped (DVDFab) to disk and fail to read others from the same DVD set ripped at the same time on the same computer.

DVDFab is set to create a .dvdid.xml file for each disk. 
Examining these files reveals no visible difference between disks that do display correctly in MC from those that do not.
Neither Season nor Disk number are present in these files.  That would have been too easy...

I poked around in some of the generated files in the VIDEO_TS folder created by the program, but could find nothing human-readable to offer a clue as to why this happens.

For reference, this particular set is the first season of Law & Order.  Six disk set.  Ripped all six one after another on my Windows 10 PC then copied the lot to the NAS, per usual.
Before the Library purge, MC displayed exactly one (blank) cover art for Law & Order and a title of "Law & Order" without season number or disk number.

Aside: L & O - UK, ripped same way on same machine displays all disks correctly.  By correctly, I mean the cover art, season, and disk number are all displayed in MC as one would expect.

Ripped file structure can be seen in attachments.
This has been a reliable process for me for the most part.  If there's a better way, please enlighten me.

Any suggestions on how to fix this rather irritating issue are appreciated.
