I just tried to create an example but was unsuccessful (problems always go away when somebody comes to fix them
). I guess I'll need to do some more research on this, but maybe somebody else has had similar problems. It occurs in the Name, Album, and Artist fields (those are the only ones I've done Find/Replace with, so it may occur in other fields as well) but it does not occur all the time.
