These will be handled better in a coming build.

Carnac will now ignore anything after a known download token like .720p or .x264.

Previously an unknown token to the right of a known token could get used in the name.

We'll also better handle the form .(token along with .token.

IMDB wants a ton of money.  Jim asked.
Most open source applications that use it just don't follow the rules and scrape the site "illegitimately".  JRiver isn't going to do that because they're a company with a checkbook that can be sued.  TMDB is really pretty good though.  As good as IMDB?  No.  Not that good, but they provide open API access with reasonable terms.

I haven't scoured this whole thread to see if it's come up, but my quickie text search didn't find this one:
t01_ type prefixes to files seem to trip up the Carnac auto-import every time. Any mkv's I rip (without manually naming) have this type of prefix. It would be nice if it could be ignored. It's got to be reasonably common too (given the popularity of MakeMKV.)

This is off the top of my head, but I think it's accurate:
N:\Media\New.Media\!Video\Movie\The Matrix\t01_The_Matrix.mkv
N:\Media\New.Media\!Video\Movie\LOTR\The Fellowship of the Ring\t01_LOTR_The_Fellowship_of_the_Ring.mkv

They don't really vary much from this.

It's the automatic numbering system that MakeMKV uses, as far as I can tell. It's naming and metadata options are pretty limited, so in the example of the Matrix any bonus features would show up as follows:
t02_The_Matrix.mkv
t03_The_Matrix.mkv

And so on, unless I specifically named them all. For movies, I almost exclusively only rip the main feature though.

correct.

t##_ is just what MakeMVK appends to the beginning of every title it rips off of a disk.  After the _ will be the same for all titles it's ripped also.

So you will end up with what bunglemebaby listed above, up to the number of titles on any one disc (I have never seen 42 available titles on any one disc, but a dozen is certainly possible).

Almost always t00_ will be the main title, but sometimes that's not the case.

I don't have MakeMKV installed on this machine, but I don't believe it will create any other file types, so yes, always mkv files

I have been downloading a lot of trailers of late (automatically) and Carnac has considerable trouble with them.  Often there is some variant on trailer in the title, e.g. -trailer, (Trailer), (Teaser trailer), (Theatrical Trailer), (Trailer) - trailer  and this seems to throw Carnac.  A manual lookup and delete of trailer and no problem.  Any chance of a tweak?
