I broke My Back And It Hurts

Saturday I tried To Stop Someone Who Was Trying To Steal Tires At A Store I Worked At. They Drove Up And Started Throwing Tires We Had On Display Into The Back Of There Truck. I Went To Try To Get The License Plate Number But The Guy Was Trying To Backup Over Top Of Me. I Did not want to get trapped In-between another vehicle that was behind me and I did not think I could get out of the way so I pulled My self Into the back of his truck. Since I was safe at that point I started to throw 5 or so tires out of his truck and after the last tire he took off and I was thrown from the back of his vehicle. I landed on my back and slammed my head into the pavement. they rushed me to the hospital and did a cat scan and found that I broke nine bones in my back. I have been on pain medication since then. I have had two appointments since then and right now they are just trying to control the pain I am having.

The company has video of what happen and I asked one of my managers if I was In trouble he said it was very heroic but I did violate company policy by confronting a customer who was stealing.

I have a fear that I will lose my job once I am released. I wish I would have just watched them take the tires, I am in so much pain right now.

Today They Got me On Oxycodone 5mg every 4hrs, And Diclofenac 50mg (anti-inflammatory), Cyclobenzaprine (Flexeril) 10mg (a muscle relaxant).

for some reason I hurt more today than yesterday.

Did they catch the guy with the truck?  I hope he's in jail.

Good luck with the recovery.  I hope you at least get some sympathy ice cream out of it.

So sorry to hear your story.

I have worked a lot in spinal units and while my background is in neurology and neuropsychology I now primarily practice in spine injuries and pain medicine rehabilitation. The following is just some general comments hopefully you will find helpful but not intending to replace your own doctors advice.

So the good news for you is that if you are walking and at home, albeit in pain, obviously the fractures must have been stable and not assoc with spinal cord damage or neurological loss. Fractures can remain very painful typically taking up to 3 months to heal and people often report very sudden relief about this time,even with vertebral crush fractures.

Controlling pain at this point is important for a number of reasons so something like oxycodone sounds appropriate and there is good evidence that adding a regular dose of simple acetominophen (paracetomol) has a synergistic effect enhancing analgesia.I dont use much by way of anti inflammatories after the first week or two and not a great fan of muscle relaxant's per se but the concurrent calming effect on the mind is sometimes useful. I tend to wean people off oxycodone as soon as practicable. Talk all this over with your doctor.

With uncomplicated fractures I advise people to keep mobile but not to overdo things,again I havent seen your CTs or examined you but assuming they are stable,uncomplicted (no neuro loss) etc. If this is the case, I spend time reassuring people that pain related to such light activity does not cause harm.This is important as otherwise a fear of activity can lead to avoidance and slower recoveries and other problems. Discuss how much activity is right for you with your doctor.

The biggest challenge many people have is with coping with the pain and its aftermath. Psychological influences like fear,uncertainty about the pain and when it will go, worry about your job, thinking about the trauma/incident over in your mind, anxiety and depression can affect your experience of pain and there is good neuroscience and evidence to support actively assessing and treating these sequelae leads to much better outcomes. I often refer my patients to specialised pain management psychologists to learn pain management strategies and to work through the various issues. Don't be shy in asking about this.It is important.

On a related note,try and avoid any medicolegal issues (if indeed relevant) if possible until you are recovered as they tend to mess with your mind unnecessarily.Try and get back to work on light duties as soon as possible. This is good for your recovery and hopefully your employer will be appreciative of your efforts.Try not to sweat it if they are not as it will only add to your suffering ( I know this is easier said than done but make the effort).

Finally when bones are healed we typically start people on graded exercise programs which boosts their confidence as well as conditions their bodies.

The vast majority of people have excellent and full recoveries so importantly try and remain positive. Remember this is a physical as well as mental challenge, recovering from trauma like this is harder than training elite athletes coz you have so much invested in it. Like athletes, however, it takes both physical and mental strategies.

My prescription for you is crank up MC with your favorite music and immerse yourself in a bit of music therapy to lift your spirits and distract from the pain a bit. Know that there are lots of us wishing a speedy recovery.

this is what the Neuro Surgeon said.

"CT: Fractures Of Spinous processes involving T3 To T11. No evidence of any other obvious fracture. CT of head unremarkable. CT C-spine shows degenerative changes but no fracture."

he said he never seen 9 of the Spinous processes broken all at the same time before.

I am trying not to take the Oxy too much, and am taking 800mg of Ibuprofen, but the Ibuprofen helps very little, I don't like taking acetaminophen, but yesterday it hurt so much I could not stand it.

I can only stand for about 30 mins at the most. If I lay down it helps, and I use a Heating pad for a bit each day.

I do take the dogs out and walk around the yard.

I go back to the Dr. this Thursday.

sorry I haven't been around in a while, over at CA causing a nuisance of myself as Audiophile Neuroscience (all the cool names were gone).

More importantly, do hope you are feeling better. people in my business talk about distinctions between pain and suffering.As counter intuitive as that first seems we know from a neuroplasticity point of view that the emotional suffering component changes the hard wiring in the central nervous system to to amplify pain pathways (central sensitisation).A bit like resetting audio gain levels.The most difficult task many people will face is being able to let go of that suffering.Here's the real counter-intuitive bit, ultimately letting go usually means coming to accept the pain.Having a strong belief that with or without the pain you will manage.Once pain loses its 'hold' over you the neurological sensitisation process begins reversing.One professor said this was the best kept secret in pain medicine and likened it to a chinese torture puzzle.The secret part was because he felt that if patients knew it they would only pretend  acceptance and fail before they start.he also thought it was a very alien concept in western culture which promotes fighting,tackling head on, never say die.As many may agree sometimes the best way to fight is by not being there and when you are,pick your battles.You can no more force pain to go away than force yourself to go to sleep, you must let go.

Of course pills, spinal blocks, exercise programs and sometimes surgery all have their place, its horses for courses.I use them all but am often reminded that the quick fixes are not always the best or lasting approach.

I actually have more pain with my arthritis now than my back, and that's something I have to talk to my GP about next month.

I think I only took 6 Oxycodone this month for my back pain (when I was having a bad day or two).

So you would be just over the 3 month mark and bones healed. Good to hear that the back pain is settling as would be expected with healed fractures. As mentioned previously soft tissue structures (including joints, muscles ,ligaments) can be pain sources and may take longer to settle. Neurological and associated Influences were mentioned in last post.

Some general view on meds. Ibuprofen is an oldie but a goodie in the class of anti-inflammatories with both pain relieving and anti-inflammatory effects. You need to take it frequently (2 or 3 times a day) and it is "non-selective" for cox 2 (you can google this but selectivity infers some theoretical advantages which may or may not be important). 800mg (3 time daily) is probably no more effective than the usual 400mg (3 times daily).Controlled studies have not shown the benefit of the higher dose regimen and warn about potential side effects (peptic ulcer, liver enzymes and cardiovascular). The maximum permissible dose used to be 1800mg/day and more recent times 3200mg per day (with anecdotal evidence only to support some patients report extra benefit...a bit like hearing differences with cables, lol).I have never prescribed beyond 1600mg/day (and typically use 1200mg per day) for reasons outlined above. Remember also in most countries you can buy ibuprofen over the counter in the 200mg dose called nurofen or advil etc.Be careful not to combine products unwittingly.

Some would say anti-inflammatories should not be required beyond a month or so post injury as inflammation (swelling etc ) has settled. But they still can be useful with or without other analgesics and especially if arthritis is also present (post traumatic or otherwise). My preferred NSAID (anti inflammaotry ) is celebrex (celecoxib) 400mg once daily. It is selective, good safety profile in comparison with others, and once daily dose.

Studies have demonstrated that regular long acting paracetamol (acetaminophen) is equally effective as an NSAID for arthritis (for osteoarthritis aka degenerative).When added to nsaids or other analgesics there is a synergistic effect meaning the sum is greater than the parts.This can mean lower doses of each med combined rather than big doses of one. It has an excellent safety profile except liver toxicity in overdose not regular doses).There has been some more recent noise around questioning its safety supremacy but most experts agree it has stood the test of time. If you have one of the "younger age" forms of inflammatory arthritis eg Rheumatoid or Psoriatic (assoc with Psoriasis) then the TNF inhibitors are now widely used.

Other things for arthritis include glucosamine with chondroitin (the combo) and fish oil. Fish oil is a good natural anti inflammatory and good for the heart (esp lowering triglycerides).Trouble is you need a lot, around 2.7g of the active ingredients (epa and dha omega 3's) per day. You need to take about 9 of the standard 1000mg fish oil tabs per day to get this. You can get concentrated liquids that provide this in 5ml.Remember add up the epa + dha and must equal over 2.5g (over 2500mg) per day. Oily fish is great but difficult to get enough epa + dha this way. Krill oil may be good but research is lacking compared to fish oil.  

Core exercise program for back is good as is walking and swimming. If swimming aggravates neck due to head turning/breathing in freestyle, use a snorkel. Some forms of Yoga are good, as is Pilates if done properly. I send people to exercise physiologists or specialized physiotherapists for a supervised rehab programs. Hydrotherapy is good for arthritis but like many things depends on the skill of the instructor. It has the advantage of being in a hot water medium which is buoyant and semi protective as not fully weight bearing but getting natural resistance from moving in water.

Back pain affects 80% of the population at some stage so maybe others will be interested in this post. Remember check with your own doctor about all the above.

I am taking MSM Methylsulfonylmethane, and Glucosamine, and Chondroitin For Joint Health, and part of the Ibuprofen is for anti-Inflammatory, and pain reasons. right now my right knee is 50% larger than my left knee. since I started taking the supplements and the Ibuprofen the swelling and pain has come down a bit.

The last time I was at the Dr's we thought it was tense muscles and was prescribed Cyclobenzapr, but now I think it is in the joints due to the swelling. Areas affected are my right knee, left elbow, neck, fingers. so last month I started to take the supplements.

If your knee is '50% larger" due to the trauma, I suggest you need an MRI.
If due to a form of arthritis then suggest you need to see a Rheumatogist(typo).. Rheumatologist

No I did not hurt my knee on the accident.

I think my knees hurt more due to my many years in the military, and crawling under houses for 15 years when I worked in pest control.

after reviewing the video they seen the guy was trying to run me over.

they told me not to climb in the back of rogue customers trucks anymore, I did not get written up.

I told them no problem I will just let them take the tires next time.

I don't like pain, and that pain that I felt I never felt before. on a scale of 0 to 10 the pain was more like 100.

"taking out" your frustrations might be as therapeutic as "letting go" ...and probably more fun (except if you're caught) ;-)
