I generally find that an MRU count of 10 seems to keep items on that list most of the time.
Tools → Options → Tree & View → Advanced → Right click recent command count
 
I've previously suggested that the MRU list be moved to the side of the menus, which would allow for more items to be displayed, while being more compact.
If you made it a little wider, it would not be difficult to add pin icons on the left.
 

 
Even without something like that, I wouldn't mind a complete menu editor though.
There's far too much bloat, and I try to avoid using anything but the MRU list now.
