After some searching and digging I've not come to a solution.
PC is a AMD 5800X 16GB RAM and Nvidia 3070. TV is LG 4K OLED.
I enabled Red October HQ, and using defaults on for high quality.
I ticked hardware accelerate and video clock to smooth playback.
Audio plays through USB DAC, Teac NT-503 ASIO. Large buffers enabled and buffering set to 100 (up from 50). Besides peak level normalize there is no DSP.
Most videos play fine, 1080 bluray is fine too and upscaling isn't a problem. With this hardware I can go insane on the settings and features in madvr and it will play it just fine.
Some videos however, seemingly regardless of settings, see frame drops and audio stutters quite badly. From the Madvr osd I see clock deviation is high, sometimes 30 or 40%. I dont know if thats a problem perse, but it seems high.
Display settings is set to automatically change mode.
I tried to disable video clock
I tried resetting madvr settings to performance, balance and quality.
The new JRVR plays the problematic videos fine (no osd to verify), but it doesn't look as good (yet) in darker scenes. I prefer some of the advanced features madvr offers.
My hardware should have no problem playing those videos, so I think its just unhappy with a setting that I cant find.
Any ideas, settings I can try to fix this?

Post OSD screen of MadVR for problematic movie.
Does it stutter through out the movie, on some scenes, or random?

I'd say its continous throughout. There can be a few seconds clean, but no more than that.

Try this
If you have a Nvidia 20xx/30xx video card and you encounter micro stutter while using madVR
without actually dropping frames, please try the following:
◦ In the Nvidia control panel, go to Manage 3D Settings, scroll all the way down and set
Vertical Sync to 'On'. You can do this either globally or just for the media player exe.
◦ In the madVR settings, go to rendering -> general settings, and uncheck 'enable automatic
fullscreen exclusive mode'
◦ In the madVR settings, go to rendering -> windowed mode settings, and set 'how many
video frames shall be presented in advance' to 1 or a maximum of 3

Oke I tried that.
It also showed me some other settings and I noticed the geforce now bar that pops up momentarily upon starting playback. I uninstalled geforce now and uninstalled the nvidia frameview sdk.
I enabled vsync to on, changed the settings in madvr as described, unfortunately it didn't help.
While looking over the 3d settings I also noticed the low latency setting. Tried that too, but no party.
I made two short movies to illustrate the problem, make sure sound is on.

Try turn off HDR in Windows
EDIT: that video you posted ... still shows present queu of 8
Try setting that to 1 as suggested

Thats weird, I changed that but i'll check again. Did you check both videos?
I'll disable HDR see what that does.

Both videos have present queu on 8.
Are you using Red October HQ or Custom with external MadVR? ... they would have separate MadVR settings
Also possible you using profiling ... and setting queue in wrong profile

I am using the default ROHQ with the quality preset from MC28. I think I hadn't saved the settings properly.
I redid the test and checked the osd to show 1 for present queue, it has the same problem unfortunately.

Can you confirm that RO Standard works? If so, it's a problem with madVR and I'll move the thread to the video board.

It is MadVR problem ... @inflatablemouse did mention that JRVR works fine

I haven't followed this thread in any detail, but some things I'd try.
Turn MC's Videoclock on and off to try it both ways.
Red October HQ without any settings changes.
The doom9 thread on madVR would also be worth trying.

Tried #1 and #2 already, the doom9 thread is 140 pages. I paged through the last 40 or 50 pages and that's all I can bear for the moment. Too much bs to sift through.

It's dense. I can't follow it. But try posting your problem. Maybe someone will give you an idea.
Customizing madVR isn't something most people should try.

Thats a beta driver for Windows 11 insider program ... i cannot help you here ... for this, you should report to insider program.
Otherwise install latest official drivers from Nvidia

Reinstalled windows 10, fully updated then ran DDU to remove Nvidia drivers and installed official 496.49.
Problem remains, on every rohq preset, I tried the suggestions above.
RO standard and JRVR work fine.
I think I'll settle for one of those then this is getting depressing.

I gave up on this and from the jrvr thread it seems madvr won't be seeing much development any more.

We could really use your help tracking this down if it affects JRVR.
We haven't made any decisions about what will happen with our madVR support. We know it's still important.

It doesn't affect JRVR or RO standard. This issue is only with some 4k videos and only when using ROHQ/Madvr.
For high quality source material I don't actually see a difference between JRVR and RO std compared with ROHQ. But in particular when upscaling from dvd or older blurays, or when the source is just bad (banding, noise) Madvr is king. Hence my desire for ROHQ as I have some relatively poor sources. For the problematic videos I don't need ROHQ as I don't see a difference, but I don't want to switch back and forth when I play a dvd or low quality source.
We haven't made any decisions about what will happen with our madVR support. We know it's still important.
I hope you make up your mind soonish. I think it would be good to manage expectations and be able to dedicate your resources towards a single choice. I also hope that will be JRVR. It looks really promising.

I tried resetting madvr settings to performance, balance and quality.
I've had similar issues in the past with madVR. Running the file "restore default settings.bat" in the C:\Users\<username>\AppData\Roaming\J River\Media Center 28\Plugins\madvr folder fixed the issue for me.

Didn't help.
What does help is disable hardware accelerate video decoding when possible.
For some reason I hadn't tried that figuring it would disable using the GPU for madvr. I'm not sure what is does now as it seems to play fine with high quality settings.
HDR looks washed though, had that problem before. Need to fiddle around with the settings some more but I'm finally getting somewhere.
I did have to increase the CPU queue size under general in madvr settings. Got a few frame drops and those short speedups if you know what I mean. Solved those by increasing them.
