This seems like a repeatable bug, to reproduce
* insert some dvd/bd to rip
* use "Get Movie/TV Info" to select a matching film which has >1 image available
* pick an image that isn't the 1st one
* rip the disc
the library item does not honour the image selection and instead always uses the 1st image in the list instead
