Title: How to Reset JRVR Settings to Default?

Does anyone know of a way to reset the video modes back to "factory settings"/out-of-the-box defaults?

Try loading one of the 'Quality Presets' Tools>Options>Video>JRVR Settings>General>Quality Presets
