I've been getting something similar since the middle days of MC11.
In my case, 60% of the time when I choose a playlist, instead of getting the cover art for the song that's playing, I always get the same album cover, (it happens to be a Crosby/Nash cover). Sometimes it will cycle thru a group of covers that still have nothing to do with the playlist. It always starts with the Crosby/Nash cover. ?
Sometimes if I restart play, the correct cover will show, but usually I have to restart MC to get that cover to go away.
Playing mp3s.

I had this before too, and solved it when I found that the offending image had been placed in the MC Cover Art folder under lots of different Artist-Album names. I think it must have been in some folder as 'folder.jpg', and got associated with all those albums.
So you need to browse the MC Cover Art folder with Thumbnail View (you could search with MC using image dimensions and file-size to limit the hits), and delete all instances of the image with the wrong name. You should also search for all 'folder.jpg's on your system and make sure there are none in a place where they could cause this problem...

There are no images in the MC cover Art folder as all my cover art is stored in the mp3 tags and the respective album folder with the mp3s. They are just stored as albumname.jpg, I do not use "folder.jpg" etc.
Edit: I went to "Images" view and the Crosby/Nash thumbnail was the first image there, I deleted it from the library and that seems to have solved the problem. ? The cover art for that album is still correctly linked.

Thanks for your answers. :)
Unfortunately my problem still remains
Can anyone tell me how to remove this image that stubbornly insists on been the default. I don't have it in my cover art file or as far as I know on my hard drive, but it still persists.
