Title: Help with track info

Would someone create a simple track info for me? I just can't get my own to work. I would be happy to pay. What I want is:

Title:  TRACKINFO_INSERT_TITLE
Artist: TRACKINFO_INSERT_ARTIST
Album: TRACKINFO_INSERT_ALBUM
Year:   TRACKINFO_INSERT_YEAR 
Genre:  TRACKINFO_INSERT_GENRE
Rank:  TRACKINFO_INSERT_RATING
         TRACKINFO_INSERT_COMMENT

Then, I would like the album art to the right. I would like a black background with the description flush left and the actual info flush left to the right of the description.
