I've discovered that I am unable to play a song by double-clicking it directly from windows explorer if MC is already open. All that happens is that MC takes focus and shows the "thinking" hourglass icon for a few seconds, but the song doesn't play or add to the playlist. If MC is NOT already open then I can play a file directly from explorer.
I've tried enabling shell integration in general settings, but this didn't help (in fact I can't see what it did at all, no extra options in windows explorer context menus)
I'm running Vista x64 and MC 12.0.380. Previously I had a problem with file associations in XP x64, which was solved by putting a copy of MC12.exe into \windows\system32. This has not helped with my current issue though.
Any suggestions would be greatly appreciated.

Have you tried rebooting the system? I'm running x86 but I've seen the same thing a couple of times and rebooting solved it evry time.

Just tested this with the current version without trouble on winxp64 SP2 on AMD. The copy of mc12.exe in system32 was necessary (in the installer in the current version).
Is the file local or on a network share?
What version of windows on what HW?

Version is 12.0.380. Running on Vista x64 on Intel. There is no MC12.exe in \windows\system32 after install, only the one in \windows\sysWOW64, but putting a copy in \windows\system32 doesn't seem to help.

Sorry, the changes were made in 389. Hopefully that build (or newer) will be available here soon.
