Title: can i connect through the internet?

Can i connect to my home MC through another computer, say with a portable MC on a thumbdrive, and stream my media? ....or do i have to be on the same network at home?

Yes you can.  You'll use Library Server.

You may need to do some network administration to get your router to let connections in from the outside world.
