Title: Question about Library Sync

Due to some other issues that are not MC related I was looking at the "Library Sync" option.
Is this a one time deal or is it possible to keep the two machines sync'd?

You can do it, but it's not perfect. Playcounts will probably come straight from the server, not added or merged. And the merging process only adds items, never deletes.
