Title: Disabled volume control & theater view OSD request

I've disabled the volume control in MC because I control volume via an external receiver. It seems reasonable to me that in this case, the Volume control on-screen display should be removed. Currently it shows "N/A" which serves no purpose and just gets in the way a little when you're navigating the menus.
