Title: How do you permanently remove entries from the Library?

I can see all the entries marked as removed in the Library using ~d=r but how to I permanently remove these entries?

Thanks - Oddly, with my "removed" smartlist (~d=r) it would show all my removed entries, I would then delete them but they would reapear once I navigated away from this smart list and back again.  If found however if I pressed the "refresh" button after deleting them, they stayed deleted....
