When converting flac to flac the first and fourth file in the queue always fails to convert.

This has been reported several times, a simple search would have found it. One thread is here (https://yabb.jriver.com/interact/index.php?topic=74614.0).

You can work around the issue by decreasing the number of simultaneous convertions to 1.
