I'd be looking at:
Tools > Options > Audio > DSP & output format > Output Format

OK, so what have you got?

If the Channels selection is 2 channel, then that's probably the answer (my guess anyway)..

Your other options are to check the manual for the receiver. Maybe there's configuring you need to do from the hardware end. Again, it's a guess..

Good luck ..  ;)

If the Onkyo says it is getting 5.1 DTS, then it most likely is bitstreaming as you want (there is no way for MC to generate DTS from 2 channels).

Congratulations on the receiver. It's actually quite a high end piece of gear, capable of 11 channels with an external stereo amp. So if you have the speakers, try out DTS Neo:X. I think you could have bought a cheaper Onkyo like the 626, 727 or 828 if you were just interested in 5.1 and 7.1 sound, though.

Can you check in Audio playback devices in Windows?
When you set the Onkyo as default device and click on configure, you should see stereo, 5.1 and 7.1 as options. Choose 5.1 or 7.1 depending on your speaker layout. I believe MC is kind of a slave to this setting.

Can you check in Audio playback devices in Windows? Where is this listed?

I have checked in sound and the Onkyo  receiver does not show, only digital SPDIF

I have noticed that my Realtek has disappeared should I reload that?

In Windows 7 you right click on the volume thing at the bottom right. Same for Vista; not sure about Win 8.
