Cable connector not fully connected can cause heat problems:

There's reports of 4090s which has been following the recommended bend guideline (and some using a harness to bend it at the recommended angle) and connected fully that are still burning up, this issue may be more widespread than thought. Personally, I think the entire adapter is faulty, thankfully I have no intention of getting a 40-series GPU, if I was, I'd get one of these (https://eustore.cablemod.com/12vhpwr-kit-picker/). :P

If you realize you didnt plug it in all the way, you might not go announce that :D

Funny thing is that no tech journalist or other expert has managed to create any kind of failure with even extreme bending or intentionally damaging the connector, unless they plugged it in weirdly.
Maybe there is some broken/low quality adapters out there, or maybe the connector is a bit harder to seat properly, and at least some of those were related to that.

For now, we don't really know yet whats the actual cause.
