I have my cover art in a single folder.  I have ripped several CD's for which there was no cover art.  I scanned the CD, and placed the file in the cover art directory.  I then added the cover art manually ("From file"). Now, when I rip another CD, as it completes, I see the "Getting cover art" dialog, which attempts to get art for the newly ripped CD, as well as several other including the four that I scanned & added manually.  After the "Getting cover art" completes, the manually added cover art has been removed from those four discs.

This is absolutely reproducible on my system.

EDIT:  It is not specific to those four CD's; I've now had several others disappear, and some of those 4 have reappeared.  I have not yet discerned a pattern for what is going on.
