OK, I have, I think figured out the problem
when ripping DVD's you get the option to rename the dvd.  However, what it appears to be doing is not renaming the file that is ripped but the description.
Example:  I ripped the dvd Robin Trower live and the name supplied was DVDVolume or something like that. No problem, blank icon appears I update tags bada bing ripped.

I then ripped Jimi Hendrix guitar lesson DVD and the name was...you guessed it....  DVDVolume, what appeared was not the blank icon but the previously ripped Robin Trower.  When I said continue....boom crash.'media center must close'
Ripped another DVD not DVDvolume, works great.

Should be an easy fix.
