With the latest build it seems like Playdoctor might be broken. 

Whether I enter in a track name or artist name it only selects tracks from a single artist.

seems to be working here. when you click on options at the right of play doctor, and click 'set rules for files that can be included'.. does that tell you something. and when you set the variety to lotts?

I have used the "Reset Settings To Defaults" and "More Variety" checked.

Are you running the latest build 127?

yes, im on 127. and I just did a reset too... and it works.
what happens when you don't add an artist or anything.. play doctor unseeded.

I had never tried "Unseeded" before.  If I did it right  (Put cursor in empty Playdoctor space and hit "enter") that seems to work fine.  but If I enter in any artist that I have a lot of such as Supertramp, Rush, Styx, Klaatu etc. etc...  It will only load / play songs by that particular artist.

If I enter in a song for an artist that I don't have a lot of it seems to vary.  Tried "Cairo" for which I have about 5 albums and every 3rd or 4th song was by "Cairo" and the other songs were divided between only 4 other artists.  Tried "Abydos" for which I only have 1 album and it seemed more random but added all tracks from that one album within the resulting list.
