mix/play doctor, basic question

i've been making car radio playlists using ~mix to get % by genre.  i've also been adding ~mix to play doctor which can be unpredictable and makes for some fun mixes.

two quick questions:  when just using ~mix, is it a truly random pick from the mix variables?  or is there an algorithm?

second:  how can i make a playlist/car radio that uses play doctor AND ~mix, and changes every time i click it?  right now i'm just using ~mix and it refreshes when i leave and come back, but i'd like play doctor to help make fun lists.  

thanks!  this is probably a very basic question, but i'm way more of an album guy but trying to make some different ~mix based smart lists so my family can listen to mixes (without say death metal getting in there) and also it's been a fun way to hear songs i wouldn't normally.  they usually listen over DLNA on the smart TV, so i want to make a refresh-able playlist since they can't use play doctor specifically over DLNA
