Missing cover art

I have transferred 1200 CD's to Media Center 19 and one simply refuses to accept cover art of any kind from any source.  Any ideas?

What file type is it?

There are six files in the album.  I have the files saved as wav, flac and m4A(ALAC).  None of them will accept the cover art.  I have tried virtually every way of attaching the cover art (of which I have a copy) but nothing works.  I have other very similar albums from the same source with no problem.

did you try doing this by individually? Try just the flac file by itself; if you selected fiferent files types embedding could be a probleme I'd imagine.

Also 6 tracks out of 1200 cds -- could be that the files are corrupted. Do they have very long file names btw?

I tried to attach the cover art to a single flac file but that didn't work.

I think you have hit on something.  These do have very long file names that were truncated when I ripped the CD.  When that happens I get a warning that says to go to the file naming place but I can't find it.
