Windows 10 - Speaker Volume/Mute

Folks,

I have noticed on ongoing issue since installing Media Center. I typically set my PC to mute (shows in system tray). Randomly, without me opening Media Center on the PC or any other media type player, the volume switches to on at some mid-level volume. Been going on a while. Anyone know if this is an issue and if so a fix?
