anything that "automatically takes me to a webpage" when I'm not expecting it irritates me badly (well intentioned or not). Instead of filling the form, I'd close the window in disgust  :D

If I uninstalled, and the uninstaller asked me if I'd consider filling out a quick multi-choice questionaire, I'd maybe click "yes".

What if I was just uninstalling to reinstall, to check/fix something? Imagine what a pain it'd be then?

I agree.  As innocent and well intentioned as this may be it will irritate some people.  I think it would be more courteous for the uninstall program to present the 'we are sorry you've chosen to uninstall...' message and to ask the user if they mind completing a form.  Out of interest, what would happen if the user was not connected to the internet at the time?  I've seen a few features of MC in the last year or so that seemed to 'assume' that every user had permanent internet access.
