I don't know exactly when this started, but over the past few weeks I've been noticing issues with Media Center changing focus to other windows in the background.
It seems to happen if a menu is visible in Media Center and I click outside of the menu to dismiss it rather than selecting something. It often clicks "through" the Media Center window to whatever window is directly underneath it, and brings that to the front.
 
Unfortunately I do not have a way to reproduce this reliably, but it's happened enough now that it's become quite annoying.
