Cannot add new tabs in MC32.0.12 MacBookPro (Intel)

While I haven't had any crashes moving tabs around in the latest version on the MBP, I now cannot add any tabs anymore. The little blank spot to the right of the tabs that I used to click for new tabs, has disappeared (see screen shot).
Also, MC now always starts up with a black tab screen and I have to click on one of the views on the right to get a tab filled with the view.

EDIT: This does not happen on the Mac Studio
EDIT 2: When I quit by <File Exit> rather than <MC QUIT> and re-open MC then a number of tags come back and the <New Tag> button is there again.
However, when I make tags and then exit, upon re-open, all the tags are the same.
EDIT 3: When I set Last location and Last exit view in <Options/Start Up> the tags come back correctly.
EDIT 4: In conclusion, closing MC using <MC/Quit> does not preserve the tabs nor the last view. However, when using <File/Exit> everything works as expected.

Are you running Media Server?

Yes, I am usually running Media Server but tab problem after quitting with <Media Center/Quit> also happens when I am not running Media Server.
