add cover art to one file only from folder

I right click on a file and choose cover art>load from file and make my choice. It changes all the art from that folder when I only want it to change the one file's cover art (it is art from a 45rpm so I don't want the whole album to have it).
It didn't used to do this and I haven't changed anything, so not sure what's going on.
Any advice on how to fix it? I've googled and can't find this specific situation.

If Tools > Options > > File Location > Cover Art > Audio mode: is set to anything other that "In a specified folder" then changing the Cover Art for one track in an album will result in the Cover Art for every track in that album being changed.
To stop it happening that option has to be set to "In a specified folder" and the "Also store image in the file's tag" option has to be switched on.

Odd behavior
I've chosen audio mode>in the same folder as file
First I deselect art, then add from file for a single file and it works....I'm thinking great
Next track, same choices but choosing art specific to this file, but this time it changes the one I'm working on and the previous one (leaving all the others in the folder alone)?
I'm stumped - this never used to happen and I didn't change anything.
One more thing - whatever I am doing isn't writing to the tag (but I have it checked to do so). When I go to the actual folder the original art is still attached.

I was typing the above reply before I saw yours. Thank you for taking the time.
It seemed that it was set to "in a specified folder" and it was changing them all, so not sure what is happening. Then it just started changing two, but not all (which makes no sense). As I said above, whatever I'm doing, it isn't writing to the file because the actual file still shows the original art (and I do have the box checked to write to file - that has never changed).

Here's my work around that works, but takes longer: I right click the file itself on my computer and choose edit tag (it is from dbpoweramp), delete original art, then select from file and I add the specific art for that file. I then go to JRiver playlist and choose update library from tag and it shows correctly without changing anything else.
I was just hoping to do it all in JRiver but there's something off and I can't figure it out.
If you have any other ideas I would love to hear them:)
