Subscription not recording every show

I have a subscription set by name for Grey's Anatomy every Thursday near 7:00PM CT.  Last night it didn't record this week's episode, and I can't figure out why.  I checked the date and time on the PC and everything looks fine.  There's plenty of space on the HDD.  I'm stumped.  I think this has happened before too, with The Good Wife, specifically.

Any ideas what's going on?

Could it be that your TV guide came with a slightly different spelling for the missed episode ?

Is the show your trying to record a "rerun"? Do you have the "do not record the show if it has already been recorded" button checked?
If so, perhaps MC thinks the show has already been recorded.

I asked in a different thread (but have not had a reply yet) regarding what MC looks for to determine if a show has already been recorded. Is it looking for a Series name and a Season and Episode number? Because some of my "OTA recordings" don't include the Season and Episode info (I suppose the info is not included in the broadcast) until I look it up and add it manually (sometimes days later).
