Blown SMT resistor/diode/capacitor near the large inductor? Hard to see if it's really missing or just a scratch on the picture...
Or is it the output GND left hanging because the black cable is the wrong way around?

I soldered the balanced input on the one channel backwards and took everything on that side to ground.

The issue had to be with something I did and yet I checked so much of the boards before looking at my own work. Odd.
