Is it possible to set up MC so that if I select a video, either from the library or Explorer, it will start playback in full screen?  I cannot seem to find any such issue.

That seems to have worked for files that in already in the library, but not for those that are accessed via Explorer.  Also, when I changed the behavior of video files every video I have gets added to Playing Now.

I'm guessing it's actually related to whether you're viewing Playing Now or not.

If Playing Now is showing, we don't jump to fullscreen.  The logic is that you can already see the display and playing in Playing Now (like double-clicking in the list) would be frustrating if it jumped.

Maybe double-check the option to General > Automatically import files when played.

You can always remove files by selecting them and using the delete command.

I have noticed that when playing a video via Explorer, Full Screen is not fully enabled (w/ only the auto-fading title bar at the top).

Rather, it is the framed full screen which requires me to close and reopen to full screen to get true full screen.
