Rip 4% and failed

Hi, I've been ripping a DVD, but one just rips 4.2% and then fails.  If I re-rip it, the rip rate is like 30mb/s, but still fails.  Can anyone explain what's causing this?  Is there a log file somewhere that might show problems?

Two directories are created during the rip.  The one having (1) in the name seems to contain subdirectories and some files.  Rip never goes beyond 4.2%.
