I think you're experiencing something related to having two copies of the same (or similar) album names in your library. To be sure, you would need to look at the files in question, directly with an external program and see if they see the cover art. Then look at the same files in MC and see if the cover art is there. I think you'll find they match. I've experienced no issues with cover art in FLAC files with hundreds of added albums in MC.

I have 2706 albums in my library. I never noticed it before but I don't usually mix audio formats. Perhaps I did not explain myself clearly enough.
- I have a cd "Load" by Metallica in my library (ALAC)
- I ripped the LP (to FLAC files), which I added as "Load LP", so the two would not mix up in the overview
- I now have "Load" (ALAC) and "Load LP" (FLAC)
- The songs from "Load LP" did not show cover art even though I had added it. (<- could it be "load LP" could not be found online so no cover was retrieved?)
- I deleted the cd rip ("Load") and renamed the vinyl rip's album title from "Load LP" into "Load"
- Cover art is now shown for the vinyl ripped files but not the cover art that I added.
- I transcoded the FLAC files to ALAC
- The cover art that I added is now shown
